Children who are 5 years old by August 1, 2023, are eligible!
Screenings only take about 30 minutes and require the child be present.
All that is required are your child's current immunization records, original or copy of their birth certificate and proof of residency -- which can be a utility, credit card, doctor or hospital bill that has your name and address of residence.
